Sagging TreatmentCombinations

Facial sagging does not have a single cause. A plastic surgeon first assesses the whole face, including fat volume, skin elasticity and skeletal support. We then consider the roles of HIFU, DENSITY, ALLTITE and injection treatments, and which combination is needed.

SAGGING MECHANISM

Where does facial sagging begin?

Diagram showing how skeletal support, skin, subcutaneous tissue and retaining ligaments relate to facial sagging

Considering skeletal and skin-supporting structures

Facial sagging is not only a change at the skin surface. We consider both changes in skeletal support and laxity of the skin, subcutaneous tissue and retaining ligaments.

  • 01Common facial changes: Hollow temples, under-eye shadows, lower cheek position, nasolabial folds, shadows around the mouth and a less-defined jawline.
  • 02Think of a tent: The skeleton acts as the poles and foundation, skin and subcutaneous tissue as the fabric, and retaining ligaments as the ropes supporting it.
  • 03How sagging develops: As skeletal support and collagen decrease, skin, subcutaneous tissue and retaining ligaments loosen, making shadows, hollows and descent more apparent.
  • 04Treatment approach: When needed, hyaluronic acid can supplement deeper support while HIFU and radiofrequency are combined to tighten the skin and subcutaneous tissue.

Assessing both skeletal support and firmness of superficial tissue, including retaining ligaments, is central to sagging treatment.

COMBINATION TREATMENT

Device features and selection

ULTRAcel Zi device used for HIFU treatment
ALLTITE radiofrequency device for facial sagging
DENSITY radiofrequency device for facial sagging
BIPOLAR RFALLTITE
Device features

ALLTITE broadly delivers radiofrequency heat from the deep dermis to the area around the SMAS. It also targets structures that support the skin from within, aiming to tighten and firm the facial and under-chin contours. Its heating method differs from HIFU, which delivers heat in focused points.

FAQ

Frequently asked questions

Can I decide not to have treatment after the consultation?

Yes. We first ask about your concerns and treatment history, and a doctor assesses your whole face. After hearing about the treatment and costs, you may choose not to proceed that day.

Why combine HIFU or radiofrequency with injections?

HIFU and radiofrequency mainly target fat and laxity in the skin and deeper tissue. Hyaluronic acid supplements contour support, while Botox adjusts muscle movement. Where several causes contribute, treatments with different roles can be combined.

How do hyaluronic acid and Botox differ?

Hyaluronic acid mainly supplements tissue volume and contour support. Botox mainly adjusts the movement of the masseter, platysma and facial-expression muscles. Their actions and suitable areas differ.

Can device and injection treatments be performed on the same day?

Sometimes, but treatment order, areas, skin reactions and previous treatments may require separate visits. We will advise you after assessment.

How are campaign discounts calculated?

Device plans have the fixed prices shown. With a device plan, Botox is 15% off and hyaluronic acid is 20% off; with both injections, hyaluronic acid is 25% off. With a three-session device course, at least four syringes and Botox, hyaluronic acid is 30% off. Without a device plan, a single injection treatment is 10% off, both are 15% off, and four or more syringes of hyaluronic acid are 20% off.

Are there additional hyaluronic acid fees?

Depending on the injection site and method, a Cattelan needle fee of ¥11,000, a forehead procedure fee of ¥22,000, or an under-eye or lip procedure fee of ¥11,000 may apply. Selecting applicable items in the price calculator adds them to the estimate; these fees are not discounted. Volux has an additional fee of ¥3,300 per syringe.

Is there downtime after treatment?

HIFU and radiofrequency may cause redness, warmth or swelling. Hyaluronic acid and Botox may cause pain, swelling or bruising. Symptoms and duration vary with the treatment and individual.

Do I need to complete all purchased course treatments during the campaign period?

No. All courses and treatments purchased during the campaign period can be used until March 31 of the following year. For a three-session course, we generally recommend treatment every 1 to 1.5 months. You do not need to complete every session during the campaign period.

Do all treatments need to be performed on the same day? Why might they be scheduled on separate days?

You do not need to have all treatments on the same day. Depending on the combination, adjusting the order and interval may help refine the result. For example, forehead Botox may be performed first, followed by hyaluronic acid injections two weeks later. When combining device-based tightening with hyaluronic acid, the device treatment may be performed first and the result finished with hyaluronic acid. Alternatively, hyaluronic acid may be injected first, with device treatment beginning one month later. Treatments purchased during the campaign period may be scheduled on separate days until March 31 of the following year.

Is payment required in full?

At your first treatment session, you must pay the full total for all courses and treatments you purchase. If you would like to pay in installments through your credit card, please check the available terms with your card issuer. Campaign prices do not apply if you pay separately for each treatment.

If I have facial-expression Botox and masseter Botox on the same day, does the campaign discount apply to each?

Yes. The applicable campaign discount applies separately to facial-expression Botox and masseter Botox. However, having these two Botox treatments together does not increase the discount rate further.
