Hyaluronic Acid Injection

Not just adding,
Support and balance for the entire face.

Nasolabial folds, hollow cheeks, the eye area, chin, and lips. Rather than just looking at the areas of concern, we examine facial features, bone structure, and changes in volume, and consider the area, layer, and amount to inject to suit each individual.

OUR APPROACH

Rather than asking where to place a single syringe, we consider where support is needed and what should be adjusted for a natural, balanced result across the whole face.

OUR POLICY

With hyaluronic acid treatment
What we value

What is important is not to decide on the product or number of syringes in advance, but to consider the necessary treatment according to each person's condition. From the examination to the post-injection process, we focus on the following six points.

  1. 01

Choose the formulation according to the site and purpose

Choose a formulation that suits the injection site, layer, and purpose, taking into account the differences in hardness and blending.

  2. 02

Explain the priority areas and amount

We will examine your entire face and clearly explain the priority areas, proposed amount, and rationale for treatment before the treatment.

  3. 03

Extra caution in anatomically complex areas

Candidacy for areas such as the forehead and around the eyes is assessed carefully because of their anatomy and risk profile.

  4. 04

    Make gradual adjustments during treatment

During the treatment, we will share the condition with you in the mirror and make small adjustments as necessary.

  5. 05

    Review the post-injection course

We will explain not only the immediate changes, but also the time required for swelling to settle and when to consider additional injections.

  6. 06

Carefully communicate individual differences and risks

We explain in advance that the duration, finish, and downtime will vary from person to person.

Information image of the director introducing the delivery results of the JUVÉDERM VISTA series for two consecutive years in the Kobe area and the characteristics of our clinic.

Two consecutive years at the top in the Kobe area for clinic deliveries of the Juvéderm series: 2024 and 2025, measured from January 1 through December 31, based on manufacturer-compiled delivery data. Reasons stated: use of longer-lasting products such as Voluma and Volux; the director clearly explains the required areas and number of syringes; extensive experience with technically demanding areas such as the forehead and around the eyes; patients can check progress in a mirror during injection; a natural change is visible immediately after injection and may be maintained for more than one year before reinjection; and the clinic carefully explains individual variation in effect and downtime as well as risks. CONCERNS

Concerns that may be treated with hyaluronic acid filler

Are you concerned about changes in volume due to age or the natural contour balance?

Nasolabial folds and shadows around the mouth are getting deeper

My cheeks or temples look hollow.

Hollows or shadows under my eyes are a concern.

I want to adjust the unevenness and roundness of my forehead

I would like to refine my chin and jawline.

I would like to discuss the shape of my lips and lip contours.

JUVÉDERM VISTA VOLUMA XC formulation package and syringe used in our clinic

Package and syringe labeling: Juvéderm Vista VOLUMA XC, injectable gel with lidocaine, two 1.0 mL syringes; product codes visible include 30354, VB20, and 453813A10; manufacturer: Allergan.

The important thing is not to just add it to the places you care about.

Even for the same nasolabial folds, the treatment required will differ depending on the cheek support, bone structure, and the location of the skin and fat. During the examination, we will check the front, diagonal, and profile views and consider whether hyaluronic acid is suitable or whether to combine other treatments.

ABOUT

What is hyaluronic acid injection?

This is a treatment in which hyaluronic acid gel is injected into an appropriate soft-tissue plane or above the periosteum to physically correct the lost volume and contour of the face. While it is easy to see changes immediately after the procedure, it may take some time for the finish to stabilize due to swelling or bruising.

At our clinic, we use the JUVÉDERM VISTA series approved by the Ministry of Health, Labor and Welfare. Each product differs in firmness and tissue integration, and is selected according to the area, layer, and purpose of injection.

The product name alone does not determine the final product. We comprehensively plan the anatomy of the face, the layers to be injected, the amount, and the location.

FACIAL AGING MECHANISM

Turn slack into a “tent structure”
Think of it as an analogy

If we compare the structure of the face to a tent, the skeleton functions as a pole that supports the entire body, the skin and subcutaneous tissues serve as fabric, and the fibrous retaining structures act like ropes to keep the fabric taut.

Explanatory diagram of sagging, comparing the skeleton to tent poles and the skin and subcutaneous tissue to fabric and supporting strings
Schematic diagram comparing the support of the skeleton to a pole, and the skin/subcutaneous tissue and support structure to fabric and string.

Three labels identify the support concept: skeletal support; skin and subcutaneous tissue; and “when support changes.” The diagram compares the face to a tent, showing how a change in the deep supporting framework affects the overlying soft tissue.

01

The skeleton is a tent pole

Just as the poles of a tent support the entire fabric from the inside, the skeleton of the face becomes the base that supports the skin and fat. Changes in the skeletal structure associated with aging are a contributing factor to noticeable depressions and shadows in areas such as the temples, under the eyes, and cheeks.

02

Skin, soft tissue, and retaining structures form the fabric and ropes

Like the fabric of a tent and the ropes that keep it tensioned, the skin, subcutaneous tissues, and fibrous support structures support the surface of the face. When changes in firmness and support overlap, they can appear as nasolabial folds, shadows around the mouth, and a less defined jawline.

03

See both support and tension

If the poles lose height or the ropes lose tension, the tent fabric will sag. Since skeletal support and surface changes overlap in the face, we assess which structural change contributes most.

The role of treatment to supplement the pole and treatment to adjust the tension of the fabric are different.

Hyaluronic acid is an option to compensate for insufficient volume and skeletal support. On the other hand, if the goal is to tighten or firm the skin, device treatments such as HIFU and RF may be candidates. Consider combinations as necessary.

TREATMENT DESIGN

Hyaluronic acid filler treatment by concern and area

Priorities are determined from overall facial balance, not from one isolated area.

01

Cheeks and temples

By supplementing lost support, shadows and contour irregularities are made less noticeable. Check not only the front view but also the connection with the cheekbone area when viewed from an angle.

02

Nasolabial folds/mouth area

Instead of injecting only into the groove, we also examine the support of the midface and the balance of the mouth. Distribute small amounts to multiple areas as needed.

03

Around the eyes/under the eyes

This area has thin skin and requires special consideration for blood vessels and nerves. Check the depression, fat, and cheek height to carefully determine suitability.

04

Chin/face line

Filler may refine the chin and lower jawline to improve balance with the profile and mouth. If sagging is the main problem, device treatment may be appropriate.

05

Forehead

Assess contour irregularity and skeletal support and consider continuity with the upper half of the face. Careful judgment is required as there is a possibility of serious complications such as incorrect injection into a blood vessel.

06

Lips

In addition to changing the size, we also adjust the amount while looking at the left-right difference, outline, and harmony with the entire face. We will share your desired shape before the treatment.

OUR FEATURES

Our approach to hyaluronic acid filler treatment

01

Look at the entire face and prioritize injections

Check changes in bone structure, left-right differences, shadows, and volume in front, diagonal, and profile views. We discuss and plan where the limited amount should be allocated in a way that looks natural.

02

Choose the formulation and layer according to the site and purpose

The required formulation and injection layer are different depending on the area areas requiring soft tissue integration and areas requiring structural contour support. Choose the one that suits your condition from the JUVÉDERM VISTA series.

Three Juvéderm Vista products are shown from left to right: VOLBELLA XC, VOLIFT XC, and VOLUMA XC. Each package contains two 1.0 mL syringes of injectable gel with lidocaine; the Japanese lines repeat the three product names.

03

Make gradual adjustments while reviewing the result

We will take a record photo before the treatment and share the design. During the treatment, we will check your condition and make adjustments, and explain the precautions to take after the injection and any symptoms that require a visit to the doctor.
