Laser treatment is not just about increasing the output. It is important to make detailed adjustments while looking at the type of problem, skin reaction, pain, and redness.
ACNE

Inflammatory acne and post-acne redness

ADVATx laser is a laser that makes it easy to consider treatment plans from multiple angles, including not only inflammatory acne but also post-acne redness, sebum amount, and skin quality. In acne treatment, it is important not only to suppress inflammation, but also to reduce redness, regulate sebum, and improve skin quality.

Studies have shown greater improvement when lasers are combined with topical treatments for inflammatory acne. Additionally, for post-acne erythema, it has been reported that redness tends to improve relatively quickly and patient satisfaction is high.

Suitable for people who repeatedly suffer from inflammatory acne, those who tend to leave redness after acne, those who want to control redness and pigmentation, and those who want to combine external treatment with device treatment.

01

Inflammatory acne

Consider using it in conjunction with topical treatment for acne with redness, acne that is prone to reoccurrence, and conditions with excessive sebum.

Research summary of ADVATx laser combination treatment for inflammatory acne
Inflammatory-acne split-face study — complete figure translation

Participants: 18 patients with inflammatory acne in a split-face comparative study. Treatment: topical benzoyl peroxide (BPO) over the full face plus a 589/1,319 nm laser on one side, four treatments at two-week intervals. Assessment: three-month follow-up.

Reduction in inflammatory-lesion count from baseline at three months: laser + BPO, 46% reduction; BPO alone, 29% reduction. The laser-combination side showed greater improvement, although the between-group difference was not statistically significant.

  • Redness and pigmentation: patients with a greater decrease in acne also tended to show improvement in pigmentation.
  • Skin unevenness and roughness: improvement was also observed.
  • Safety: no serious adverse events were reported.

The percentages describe this study population and do not guarantee an individual result. No fee is printed in the figure.

02

Redness of acne scars

If redness remains even after the inflammation has subsided, consider the type of irradiation while looking at the vascular reaction and skin quality.

Research summary of ADVATx laser treatment for post-acne erythema
Post-acne erythema split-face study — complete figure translation

Participants: 29 patients with post-acne erythema in a split-face comparative study. Treatment: 589/1,319 nm laser on one side and aloe-vera (AV) gel on the other. Assessment: after six treatments with an eight-week follow-up; AV gel was applied twice daily for 18 weeks.

A. Improvement by at least one grade at week 8: laser side 72%; AV-gel side 69%. The percentage axis is marked 0%, 25%, 50%, 75%, and 100%. B. Speed of improvement: the laser side showed significant improvement from week 2; the AV-gel side from week 4. The laser side tended to improve sooner, but no between-group difference was shown at the final assessment.

  • Relationship to acne: improvement in redness correlated with improvement in acne severity (rs = 0.47, p = 0.03).
  • Patient satisfaction: tended to be higher on the laser side.
  • Safety: no serious adverse events; pain was mild, with mean VAS 1.52/10.

These findings describe the study population and do not guarantee an individual result. No fee is printed in the figure.

SKIN AGING

How skin aging develops

The skin is made up of two major layers: the epidermis and the dermis. The epidermis acts as a barrier to protect the skin from external stimuli and dryness, and the basal layer contains melanocytes that produce melanin. The dermis contains collagen, elastin, capillaries, immune cells, etc., and supports the skin's firmness, elasticity, complexion, and temperature regulation.

Illustration of skin aging showing the two-layered structure of the epidermis and dermis, melanocytes, and capillaries.
Skin-layer labels in the Japanese diagram
  • Epidermis: stratum corneum, granular layer, spinous layer, and basal layer; the basement membrane lies beneath the basal layer, where melanocytes are also marked.
  • Dermis: superficial and deep dermis, with capillaries shown among the fibers.
  • Deeper layer: subcutaneous tissue below the dermis; the panel at left is an H&E-stained skin section.

Due to the effects of age, UV rays, friction, inflammation, etc., turnover in the epidermis decreases and melanin excretion tends to be delayed. Additionally, melanocytes become hypersensitive to stimuli, making melasma and pigmentation more likely to worsen. Furthermore, when the basement membrane that connects the epidermis and dermis becomes flattened, torn, or becomes multilayered, the barrier function decreases and the skin becomes susceptible to continued inflammation.

In the dermis, collagen and elastin production decreases due to decreased fibroblast function. On the other hand, decomposition progresses more easily and the dermis becomes thinner. As a result, the capillaries move closer to the surface, making redness more noticeable, the skin becoming less firm, and sagging more likely.

An explanatory diagram comparing changes in the basement membrane of young skin and aging skin.
Comparison of skin sections at ages 17 and 87
  • Age 17: the basement membrane has a wavy, undulating contour.
  • Age 87: the diagram labels flattening, breaks, and multilayering of the basement membrane; adherence of the stratum corneum; flattening and atrophy of the epidermis; basal-layer pigmentation; and solar degeneration in the dermis.

In other words, skin aging is not just a problem of age spots and sagging, but is also linked to redness, inflammation, melasma, and pigmentation. As fibroblast function declines, the dermis becomes thinner and the basement membrane and barrier function weaken, making it easier for micro-inflammation to continue. This inflammation leads to dilation of capillaries and stimulation of melanocytes, which can lead to chronic redness, melasma, and worsening of pigmentation.

An illustration of the occurrence of decreased fibroblast activity, telangiectasia, and chronic inflammation in the aging dermis.
Age-related cycle shown in the dermis

From the epidermis and basement membrane through the dermis and subcutaneous fat, the diagram shows a possible cycle associated with aging and repeated stimulation: reduced fibroblast activity; reduced collagen and elastin production; loss of elasticity and thickness; dilated capillaries; chronic inflammation; and greater susceptibility to dullness and pigmentation.

REDNESS · MELASMA

Relationship between redness, melasma, and skin aging

Skin redness is not simply a matter of surface color, but reflects photoaging and chronic inflammation of the dermis. When the structure of the dermis changes due to the effects of ultraviolet rays, friction, aging, etc., capillaries tend to dilate, resulting in a skin condition where redness tends to persist.

Redness is a sign of changes in the skin's foundation and reflects photoaging and chronic inflammation in the dermis.
Figure 1 — Redness can signal changes in the skin foundation

Redness may reflect photoaging or chronic inflammation in the dermis. Ultraviolet exposure, friction, and aging can make capillaries more likely to dilate. The diagram labels the epidermis, dermis, and subcutaneous tissue and compares normal with dilated capillaries.

When examining age spots and melasma, it is important to not only look at the pigment, but also to check for redness and inflammation. In skin with strong redness, inflammation stimulates melanocytes and increases melanin production, making melasma and pigmentation more likely to worsen. Therefore, "pigment treatment" alone may not be sufficient for melasma. In addition to toning and whitening treatments, it is important to calm redness, inflammation, and telangiectasia, and prepare the foundation of the skin.

Illustration that redness and inflammation stimulate melanocytes, which tends to worsen melasma and pigmentation.
Figure 2 — Redness may aggravate melasma

The arrows show this sequence: persistent redness and inflammation → stimulation of melanocytes → increased melanin → more visible melasma and pigmentation. Prolonged inflammation may make pigment concerns more persistent.

Rosacea and redness are associated with decreased skin barrier function, dilated capillaries, and chronic inflammation. Treatment may include a combination of skin care and lifestyle changes to avoid irritation, as well as treatments that target redness and inflammation.

Melasma is an explanatory diagram in which it is important to consider not only pigment but also redness, inflammation, telangiectasia, and the preparation of the skin's foundation.
Figure 3 — Melasma may require more than pigment treatment
  • Pigment: address melanin production and deposition.
  • Redness: address redness associated with dilated capillaries.
  • Inflammation: reduce inflammation and support the skin environment.
  • Skin foundation: support the condition of the dermis.

The four perspectives are considered together to improve overall balance; this may help treatment planning and recurrence prevention.

In the ADVATx laser, 589 nm is sensitive to hemoglobin and is used to treat redness, inflammation, and telangiectasia. For melasma with redness, redness on the face, redness from acne scars, etc., we consider treatment strategies that include not only pigment but also redness control. 1,319 nm stimulates fibroblasts, increases the production of skin collagen and elastin, strengthens the dermis, reduces inflammation and improves barrier function.

The 589 nm wavelength is readily absorbed by hemoglobin and may be used for redness, inflammation, and dilated capillaries. The diagram labels the epidermis, dermis, and subcutaneous tissue and illustrates selective treatment of a dilated capillary. Redness-associated melasma, facial redness, and post-acne erythema may be included in the treatment plan.

SKIN BOOSTING

What is skin-quality treatment?

In skin education, an explanatory diagram that compares the skin to soil and changes to fruit.
1 — Skin as soil and visible change as fruit

The diagram compares the skin foundation to soil and visible outcomes—brightness, firmness, smoothness, and refined texture—to fruit. Dry, hardened soil, insufficient nutrients, and nutrient imbalance are shown as conditions less likely to produce good results, emphasizing assessment and preparation of the skin foundation.

Hadaiku is a way of thinking that does not only focus on results such as whitening, firmness, smoothness, and fine texture, but also prepares the foundation of the skin that will produce those results. The materials compare skin to "soil" and changes such as whitening and firmness to "fruit." Just as it is difficult to grow good fruits in soil that is dry and hard, lacks nutrients, or is out of balance due to over-nutrition, it is important to assess the current condition of the skin and take care of it.

Hadaiku is an explanatory diagram that combines nurturing treatment and nourishing treatment.
2 — Combine stimulation and nutrient-delivery approaches

After examining the current skin condition, treatment may combine approaches that stimulate the dermis, such as laser or radiofrequency, with approaches that deliver selected ingredients, such as MESONA-J or injectable treatment. The sequence and combination depend on the findings.

Hadaiku combines ``cultivation treatment'', which applies moderate stimulation to the skin to elicit a response in the dermis, and ``nourishment treatment'', which delivers necessary ingredients to the skin. Lasers and high frequencies play the former role, while MESONA-J and injectable treatments play the latter role.

Diagram of the dermis and fibroblasts, which are central to skin care
3 — The dermis and fibroblasts

The diagram separates the epidermis and dermis. Purple cells represent fibroblasts, red fibers represent collagen, and blue fibers represent elastin. Fibroblasts participate in producing collagen and elastin and help maintain skin firmness; skin-quality treatment aims to support a healthier dermal structure.

The core that supports the foundation of the skin is the dermis. In the dermis, fibroblasts are involved in the production of collagen and elastin, supporting the firmness and elasticity of the skin. The goal of skin education is to revitalize these fibroblasts and restore a healthy dermis structure. ADVATx laser can be used to stimulate the dermis and fibroblasts through the thermal action of 1,319 nm, and is an option when considering improving skin quality, firmness, shine, pores, and sebum balance.

An explanatory diagram of skin care treatment that combines ADVATx laser, MESONA-J, injectable treatment, and ALLTITE.
4 — Combine treatments according to the skin condition

Around the goal of supporting the skin foundation, the diagram lists ADVATx, MESONA-J, injectable treatment, and ALLTITE radiofrequency treatment. The ADVATx panel specifically identifies the 1,319 nm wavelength and states that it is used with the aim of stimulating the dermis and fibroblasts. Whether to combine treatments, and their order and interval, are decided from the examination.

Skin-quality care may require more than one type of treatment. Depending on the skin condition and treatment goal, a plan may combine ADVATx, MESONA-J, injectable treatment, ALLTITE, or other appropriate options. Improved brightness or firmness may follow from addressing dermal condition, inflammation, dryness, sebum, and barrier function in a coordinated plan.
